Job summary

Chrysalis Health in Florida is seeking a Mental Health Targeted Case Manager to support individuals and families across diverse age groups. You will coordinate services, collaboratively develop treatment plans, and connect clients with community resources to promote wellbeing.

Ideal candidates hold a bachelor's degree in a related field, have at least one year of experience with youth with emotional disturbances, and can document care in an EMR system.

Qualifications

• Must reside within the State of Florida.
• Bachelor's degree from an accredited university in Psychology, Social Work, MFT, Mental Health Counseling, or related field.
• Ability to document therapy services in an EMR platform.
• One year of experience working with children with serious emotional disturbances.

Responsibilities

• Provide individual and family therapy to a diverse population.
• Document therapy services in EMR.
• Coordinate care and connect clients with community resources.
• Support professional growth and progression within Chrysalis Health.

Candidate Requirements:

• Must reside within the State of Florida
• Education: Bachelors degree from an accredited university or college in Psychology, Social Work, Marriage and Family Therapy, Mental Health Counseling, or related field.
• Compassionate, engaging, and have a strong desire to help others.
• Ability to document therapy services in electronic medical records (EMR) platform.
• One year of experience working with children with serious emotional disturbances
