We are seeking an enthusiastic and highly-motivated Mental Health Support Worker to join our Dementia Outreach Team and be a part of a service that really makes a difference to patients experiencing a dementia-related crisis, and their families. The successful candidate will provide day to day care for service users under supervision of a Clinician which adheres to best practice, standards and guidelines, and contributes towards the service users care and treatment.
You will work in partnership with service users, other members of the multidisciplinary staff as appropriate, to achieve the optimum health outcome for clients.
Your work will involve working with elderly people who are suffering a Dementia-related crisis. Your role will include enabling people to stay living as independently as possible, to take part in activities in their local community and supporting them to make a good recovery and maintain mental and physical wellbeing. We are looking for well-motivated, conscientious and friendly people who are willing to learn and to play a key role in improving the lives of the patients we work with. You will have contact with patients, relatives, other members of the public and professionals from partner organisations.
