Job summary

GMA Interventions seeks a dedicated professional to provide direct-care services to adults with serious mental illness in home and community settings. Under supervision, you will assess risk, coordinate care, and support treatment goals, delivering up to 15 hours per client.

You’ll collaborate with a multidisciplinary team, prepare service plans and progress notes, respond to crises, and help stabilize clients in their communities while maintaining compliance with mental health regulations.

Qualifications

• Bachelor's degree in social work, psychology, counseling, or related field.
• Must meet the QMHP A or QMHP T criteria.
• At least 1 year of experience in the mental health field.

Responsibilities

• Assess clients' risk levels and follow initial assessment-based plans.
• Coordinate care with other service providers to meet objectives and identify supports.
• Prepare written individualized service plans and treatment summaries (quarterly/monthly).
• Plan and coordinate services to improve independence and stability in the community.
• Respond to crises 24/7 availability.
• Complete progress reports within 24 hours of service delivery.
• Collaborate with physicians, therapists, psychiatrists, case managers, and substance abuse providers.
• Refer clients to other agencies for treatment/services to aid recovery.
• Monitor, evaluate and record client progress toward treatment goals.
• Participate in supervision and training offered by the agency.

Job description

Under the direction and supervision of the Clinical Supervisor and Program Director, this position provides an array of training, support and activities of daily living services to individuals diagnosed with severe mental illness in the community setting. The position will deliver direct-care services (up to 15 hrs. per client) to support clients with mental health problems within their home and community.

Duties and Responsibilities

• Consistently assessing client’s levels of risk, and carrying out their needs based on initial assessment based on Mental Health Skill Building 7 Core competencies.
• Facilitates care coordination with other service providers to enlist existing supports in determining and meeting service objectives and to identify additional resources and supports
• Prepares written individualized service plans and treatment update Summaries-Quarterly and/or Monthly, incorporating input from clients, other service providers, wraparound team members to identify problem areas and needs, service objectives, and intervention strategies
• Planning, recommending and coordinating services that improve the level of independence and stability in the community to prevent substance abuse and to combat social problems.
• Responds to crisis situations on a twenty-four hour a day availability
• Completes progress reports in reference to service delivery within 24 hours
• Collaborating with physicians, therapist, psychiatrist, case manager, substance abuse provider to develop, plan and manage treatment.
• Referring client to other agencies for treatment/services to help in recovery from physical or mental illness.
• Monitoring, evaluating, and recording client progress with that of treatment goals.
• Participate in case supervision and training offered by agency

Education and Qualifications

• A Bachelor's Degree in Social Work, Psychology, Counseling, or a related field from an accredited college or university with a minimum one year of experience in the mental health field. Must meet the criteria to be a Qualified Mental Health Professional (QMHP A or T).
