Job summary

Allina Health is seeking a Mental Health Practitioner for the Adult Day Treatment and related programs in Minnesota. The role provides direct and indirect patient care under supervision, with a focus on therapeutic activities, discharge planning input, and adherence to CMS/MN guidelines.

The position emphasizes collaboration across disciplines and patient-centered planning. The candidate will participate in supervision and follow safety procedures, with a .6 FTE commitment, 8-hour day/evening

Qualifications

• Bachelor's degree in OT/therapeutic recreation/music therapy/dance therapy or social/psychological area as required.
• Meets MH Practitioner Qualifications in accordance with MN § 245I.04, Subd 4.

Responsibilities

• Assessments and reassessments of patients.
• Plan and conduct educational/therapeutic activities.
• Document assessments and contribute to treatment planning.
• Coordinate care with MH professionals and supervisors.
• Support patient rights and safety in all settings.
• Engage in supervision under a signed plan.
• Other duties as assigned.
