Merakey is seeking a Mental Health Professional in Lake Charles, LA, to join our Assertive Community Treatment (ACT) Team. You will deliver individual and group therapy, develop treatment plans, and ensure thorough psychosocial assessments with timely clinical documentation.

The role emphasizes crisis intervention, support, and collaboration within a multidisciplinary team. Weekly hours are based in a community setting with competitive benefits and mileage stipend.

• Must have an active PLPC/LMSW within the state of Louisiana.

Responsibilities

• Provide individual and group therapy to consumers in service.
• Create and review person-centered treatment plans and crisis plans.
• Complete psychosocial assessments and maintain timely clinical documentation.
• Provide crisis intervention and ongoing support.

Earn $26.44/hour + $7,500/yearly mileage stipend

The Mental Health Professionals (MHP) are an essential part of the Assertive Community Treatment (ACT) Team. The multidisciplinary team provides evidenced based services to members diagnosed with severe and persistent mental illness. The mental health professional provides individual therapy and group therapy to consumers in service. The MHP creates the person-centered treatment plan, review and change the individual crisis plan as required, and complete psychosocial assessments. Must maintain thorough and timely clinical documentation such as: progress notes, assessments, and interpretative summaries. MHPs provide crisis intervention and support.
