Job summary

Merakey is seeking a Mental Health Professional to join our Lafayette, LA location. The role supports the Assertive Community Treatment (ACT) Team, delivering individual and group therapy, and developing treatment plans for clients with severe mental illness.

A Louisiana PLPC or LMSW is required, with an active driver's license. The position includes mileage stipend and full benefits.

Qualifications

• Must have an active driver's license within Louisiana.
• Willingness to work in a community-based setting.
• Active PLPC or LMSW licensure in Louisiana required.

Responsibilities

• Provide individual therapy and group therapy to ACT clients.
• Develop, review and update person-centered treatment plans.
• Complete psychosocial assessments and crisis plans as required.
• Maintain thorough clinical documentation (progress notes, assessments, summaries).
• Provide crisis intervention and supportive services as part of the ACT team.

Position Details

The Mental Health Professionals (MHP) are an essential part of the Assertive Community Treatment (ACT) Team. The multidisciplinary team provides evidenced based services to members diagnosed with severe and persistent mental illness within the greater Baton Rouge. The mental health professional provides individual therapy and group therapy to consumers in service. The MHP creates the person-centered treatment plan, review and change the individual crisis plan as required, and complete psychosocial assessments. Must maintain thorough and timely clinical documentation such as: progress notes, assessments, and interpretative summaries. MHPs provide crisis intervention and support.
