Keystone Human Services seeks a Lead Intensive Case Manager to guide a team and ensure high-quality, person-directed support for adults with mental health needs. You will mentor fellow case managers and review case notes to maintain standards and compliance.
Beyond supervision, you will help clients access medical, social, educational, and additional resources, contributing to a supportive, inclusive community that aims for growth and independence.
Keystone Human Services is currently seeking a Lead Intensive Case Manager to join our team in providing individualized, person-directed support in the area of mental health. Our work focuses on inclusion and building communities where everyone has the opportunity to thrive and fully participate in everything society has to offer. As a Lead Intensive Case Manager, you will assist adults with mental illness gain access to needed resources such as medical, social, educational, and other services. This position mentors other Intensive Case Managers on the team and assists in case note reviews and approvals. Keystone Human Services values health, wellbeing, and professional growth. Job Details: Full time, non-exempt position $24.00 per hour
(one of the following) Master’s degree in social work, psychology, rehabilitation, activity therapies, counseling, or education and three years of direct care experience in mental health Bachelor’s degree in nursing and three years of direct care experience in mental health Bachelor’s degree in social work, sociology, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling, or education; or be a registered nurse; and five years of direct care experience in mental health including two years of supervisory experience
Effective communication and proficient computer skills Ability to resolve complex issues and respond effectively in crisis situations Valid driver’s license with daily access to a privately maintained and insured vehicle Successful completion of the pre-employment process, including clearances and verifications
