Responsibilities

• Facilitates therapy sessions for individuals or groups.
• Develops individualized service plans focusing on client and family needs.
• Monitors progress and participates in service plan reviews.
• Updates service plans and submits them for medical review as applicable.
• Conducts intake, including explaining services and collecting information.
• Writes assessment summaries and coordinates with resources as outlined in the service plan.
• Maintains contact with referral sources and provides progress updates.
