Job summary

• Master's degree in a human services related field.
• Experience providing mental health services preferred.
• Certification/Licensure: N/A

Responsibilities

• Facilitates therapy sessions for a client or group of clients.
• Assists client in developing an individualized service plan, monitoring progress and updating plans as needed.
• Conducts intake and assessment; documents and submits materials for processing.
• Assists clients in linking with internal and external resources per service plan; maintains referrals and progress reports.
• Assists with discharge planning and follow-up services.
