Description

The Children's Community Support Collaborative (CCSC) is a program of The Home for Little Wanderers located in a recently constructed, state of the art facility overlooking the Boston skyline in the Roxbury Crossing neighborhood of Boston. The Collaborative is a Department of Mental Health contracted program, that provides Intensive Home-based Therapeutic Care services to youth throughout Boston to help them remain safe in their homes and communities. The program also offers Therapeutic Group Care services for young people who need additional support. The program is seeking a master's level Clinician to provide family therapy and intensive case management.


The Children's Community Support Collaborative (CCSC) is a program of The Home for Little Wanderers located in the Roxbury Crossing neighborhood of Boston. We provide Intensive Home-Based Therapeutic Care services to help families remain together, rebuild connections that have been broken, and provide a unique group home treatment environment for clients not able to remain in their homes.


We offer a highly supportive, well resourced, and flexible approach to care. Weekly supervision, multidisciplinary teams, and intensive clinical training support not only the best possible outcomes for our families, but a work environment where the collective wisdom and approaches of all the supporting disciplines lead to amazing outcomes for kids and families. Our multi-disciplinary team includes occupational therapists, peer specialists, outreach workers, milieu staff, nurses, psychiatrists, clinicians and administrators, and offers many opportunities for connection, collaboration, growth and a collegial atmosphere.


We are committed to trauma-informed care that centers the voices of youth and families. To this end, we use principles and practices from Integrative Treatment of Complex Trauma (ITCT), Therapeutic Crisis Intervention (TCI), Restorative Practices, and Permanency work to support family stabilization. We focus on providing age-appropriate, culturally sensitive therapy for clients and families in a variety of settings both in the community as well as within out Therapeutic Group Care treatment service.


How You'll Be Making a Difference



  • Under the direction of the Clinical Coordinator, the Clinician will provide family therapy, case management, and group therapy.

  • Act as a liaison with collateral providers.

  • Establish and maintain community-based services that support the growth of youth and families.

  • Advocate for families' needs and teach caregivers to advocate for themselves and their children.

  • Help families develop the skills needed to navigate the mental health system.

  • Perform initial and ongoing assessment of youth and families.

  • Design treatment plans that address areas of need and specify intended outcomes.

  • Work as part of an interdisciplinary team to provide quality care.

  • Attend staff meetings and case conferences, present client material.

  • Facilitate communication between various providers.

  • Submit reports, documentation and billing in a timely manner.

  • Ensure that documentation meets contractual, utilization review and quality assurance standards.

  • Attend professional development opportunities to enhance and develop clinical skills.

  • May provide training, workshops and consultations to teachers, parents and collateral staff.

  • Perform program specific duties and functions.

  • May participate in agency committees.

  • Provide crisis intervention as needed.


Qualifications



  • Master's degree in social work, mental health or related discipline.

  • May be licensed as in independent practitioner in the state of Massachusetts.

  • May or may not have program specific experience.

  • Excellent cultural awareness and sensitivity.

  • Computer literate, including Microsoft Office and Internet Explorer, with the ability to learn new software applications.
