Description
(General description and summary)
The Mental Health Clinician (MHC) provides clinical and supportive services to participants within the Compass Rose program as part of a multidisciplinary team. Compass Rose is a supportive housing program offering clinical services through Medicaid and wraparound supports via a contract with DHS, targeting transitional aged youth 18-21 who are aging out of foster care. The position involves developing, implementing, and monitoring individualized treatment plans that specify medically necessary services for treatment and recovery. It oversees skills training, case management, counseling, and other services in both clinic and community settings. Significant administrative responsibilities include documentation, reauthorization, and other routine tasks.
Responsibilities
- Case Management
- Develop relationships with clients through scheduled meetings for case management and counseling.
- Assess strengths and service needs of each participant.
- Create individual service plans collaboratively with participants and supports.
- Assist participants in connecting with resources and services to meet their goals.
- Monitor service delivery and coordinate care.
- Build relationships with providers and stakeholders to facilitate services.
- Ensure scheduling and attendance of medication management visits and address issues to ensure effective medical care.
- Provide customer service to stakeholders.
- Counseling
- Provide behavioral health counseling in individual and group settings.
- Assess and monitor therapeutic progress, ensuring services remain necessary and aligned with assessments and plans.
- Utilize evidence-based practices such as motivational interviewing, CBT, DBT, wellness management, and dual diagnosis treatments.
- Crisis Intervention
- Implement trauma-informed care principles to minimize crisis service use.
- Coordinate respite entry and complete crisis plans.
- Conduct risk assessments and safety planning.
- Collaborate on proactive steps to reduce crisis interventions.
- Document hospitalizations and ER visits, assist with discharge planning, and follow reporting guidelines.
- Maintain a crisis phone on a rotating basis.
- Administration
- Complete assessments, reauthorization, and funding documentation.
- Document all contacts within 72 hours.
- Develop clinical service plans, reviews, and progress reports.
- Ensure compliance with federal, state, and county regulations.
- Meet productivity expectations of at least 50% billable direct services.
- Collaborate with internal teams and external networks.
- Utilization Reviews
- Provide requested information and ensure documentation compliance.
- Participate in supervision and review interventions with supervisors.
Additional Expectations
- Participate actively in meetings.
- Maintain familiarity with local resources.
- Pursue ongoing education (minimum 16 hours annually, including 12 in mental health/dual recovery and 4 in cultural competency).
- Follow policies, procedures, and ethical guidelines.
- Maintain required credentials (e.g., QMHP or equivalent), current driver’s license, CPR, and First Aid certification.
- Adhere to a set work schedule and follow directives.
- Ensure the security of all corporate interests.
Qualifications
- Masters Degree in social work or related field with QMHP credential.
- At least two years post-graduate experience preferred.
- Experience with trauma-informed care and bilingual capabilities are advantages.
- Knowledge of DSM V, mental health assessments, and service planning.

Requires extensive mental activities, including judgment, communication, calmness in crises, empathetic listening, and analytical skills. Must motivate others, be flexible and creative in problem solving, and collaborate effectively.
