Amae Health in Raleigh, NC is hiring a Clinical Care Coordinator to support members’ daily experiences and sense of belonging across Raleigh and Charlotte clinics. You’ll facilitate groups, wellness programming, and engagement activities while collaborating with psychiatrists, therapists, dietitians, and primary care providers to ensure coordinated member care.

Qualifications

• Enrolled or recently graduated in a degree program related to mental health.
• Interest in mental health and desire to work with members with Severe Mental Illness.
• Strong interpersonal skills and ability to work in a team environment.

Responsabilités

• Facilitate groups, arts and crafts, and recreational activities to enhance the member experience at Amae Health.
• Welcome members to the clinic, manage communications via phone and email, and assist with scheduling and member record updates.
• Collaborate with psychiatrists, therapists, dietitians, and primary care providers to share observations and support coordinated member care.
• Identify members’ ongoing needs and help connect them with community resources and supports that promote wellness.
• Contribute to projects that support clinic growth and ongoing member success, and participate in training to build skills in mental health support and community resources.
