Job summary

Amae Health, a Series B venture-backed Public Benefit Corporation, delivers specialty outpatient care for people with severe mental illness through high-acuity clinics nationwide.

As a Clinical Care Coordinator, you’ll support members’ daily experiences across San Mateo and Los Altos, facilitate groups and wellness activities, manage communications and records, and collaborate with psychiatrists, therapists, dietitians, and primary care providers to ensure coordinated, compassionate care.

Qualifications

• Enrolled or recent graduate in a mental health-related degree program.
• Interest in mental health and working with Severe Mental Illness.
• Strong interpersonal skills and ability to work in a team.
• Detail-oriented and organized.
• Comfortable one-on-one or in group settings with excellent interpersonal skills.
• Willingness to collaborate with others and plan events.
• Basic computer skills required.
• Holistic health perspective and ability to work with diverse backgrounds.

Responsibilities

• Facilitate groups and recreational activities to enhance member experience.
• Welcome members, manage communications, and assist scheduling and records.
• Collaborate with interdisciplinary care team to support coordinated member care.
• Identify members' ongoing needs and connect them with community resources.
• Contribute to clinic growth and participate in training to build skills.
