Job summary

Transitional Services for New York, Inc. (TSINY) seeks a Case Manager for its Jamaica, Queens residential program.

You will coordinate assessment, treatment planning, and care coordination for residents, advocating for their rights and supporting transition to independent housing. The role requires a bachelor’s or associate degree (or HS with experience) in mental health, valid driver’s license, and strong communication.

Qualifications

• Bachelor’s degree or Associate’s degree plus one year experience or HS diploma plus two years in mental health setting.
• Valid Driver License Required.
• Ability to work independently and take initiative.
• Strong oral and written communication and interpersonal skills.

Responsibilities

• Coordinate and monitor assessment, treatment planning and care for each resident.
• Advocate for residents’ rights and coordinate with treatment providers.
• Develop person-centered Service Plans and monitor progress.
• Link residents to community resources and housing options.
• Monitor medications and provide crisis management when required.
• Collaborate with interdisciplinary team and visit hospitalized clients.
