Qualifications

• Bachelor's degree or Associate degree plus one year of experience in a mental health setting.
• High School diploma plus two years of experience in a mental health setting.

Responsibilities

• Provide case management and counseling to individuals.
• Assess needs, make referrals, and link individuals to services.
• Follow up with discharged individuals and assist with transportation to appointments.
• Maintain case records in accordance with DOHMH regulations and TSINY policies.
• Escort participants to appointments as needed.
• Engage in outreach to educate and enroll participants into services.
• Coordinate discharge planning and post-discharge follow-up.
• Attend monthly staff meetings and weekly supervision.
