Job summary

Phoenix Behavioral Health Services, LLC is hiring a full-time Case Manager/Group Counselor for our Mental Health Partial Care program in Ewing, New Jersey. The role includes leading psycho-educational groups and managing a caseload with detailed documentation of progress notes and treatment plans.

Responsibilities also include biopsychosocial evaluations, outreach calls, and coordinating case management for clients with mental health and co-occurring needs.

Qualifications

• Experience in behavioral health treatment settings is required or preferred.
• Must have experience providing case management and group treatment.
• Strong communication skills and ability to work on a multidisciplinary team.

Responsibilities

• Lead in-person psycho-educational groups.
• Manage a designated caseload and document progress notes.
• Complete individual service/treatment plans for clients.
• Prepare biopsychosocial evaluations and outreach calls.
• Coordinate case management needs for clients with mental health and co-occurring issues.
