Qualifications

Responsibilities

• Provide intensive community-based services to consumers.
• Coordinate and integrate services from multiple providers.
• Maintain accurate consumer clinical records in an electronic format.
• Provide on-call crisis intervention and collaborate with the Clinical Team.
