Job summary

Qualifications

• Valid driver’s license is required.
• Emergency behavior management training within 90 days of orientation.

Responsibilities

• Complete a comprehensive assessment of the consumer to determine needs and services.
• Develop an action plan with the consumer to reach service goals.
• Provide crisis assistance when immediate attention is needed.
• Assist with daily living tasks to help achieve independence.
• Provide linkage, referrals, advocacy to access resources.
• Maintain records and documentation per guidelines.
• Meet productivity and timely paperwork standards.
• Interact with clients with varying mental health challenges.
