Job summary

West Tennessee Healthcare in Milan, Tennessee is seeking a Case Manager to coordinate mental health services for a designated caseload. This eight-hour day position includes on-call duties as needed and requires collaboration with the Clinical Manager to meet client goals.

Qualifications

• Bachelor’s degree in a behavioral health-related field or RN license required.
• If bachelor’s degree is in another area, 15 college credits in behavioral health or 1 year of behavioral health experience is required.
• Valid driver’s license and emergency behavior management training within 90 days of orientation.

Responsibilities

• Assess consumers to develop an action plan and coordinate services.
• Provide crisis assistance and linkages to community resources.
• Maintain required records and ensure timely documentation.
• Engage with mentally ill or emotionally disturbed individuals to promote safety and independence.
• Participate in QA/I, Utilization Management and related agency functions.
