Job summary

Child & Family Focus, Inc. is seeking a Blended Case Manager to develop and coordinate services for children and families per the service plan.

You will provide case management, service planning, and coordination with other agency providers to meet client needs, along with supportive counseling and advocacy to empower self-advocacy. Travel across Delaware County is required to meet clients face to face, using IPAD and phone for documentation and outreach.

Qualifications

• Must have 12 credits in Human Services or Psychology or related field.
• At least 1 year of direct clinical experience with children, youth and families.
• Ability to work Monday–Friday 8:30am–8:30pm with 3 evenings per week.

Responsibilities

• Develop, coordinate, and implement services per the service plan.
• Provide case management, service planning, and coordination with other providers.
• Advocate to help clients set and achieve recovery goals and increase self-advocacy.
• Complete home, school, hospital and community visits; travel across Delaware County.
• Utilize IPAD and cell phone to document and coordinate resources.
• Participate in weekly team meetings and supervision.
• Manage caseload up to 30 clients and meet billable hour requirements.
• Provide 24/7 on-call crisis support in rotation with staff.

Job description

The Blended Case Manager is responsible for developing, coordinating, and implementing services for consumers as identified in the service plan. Responsibilities include providing case management, service planning, and coordination of services with other agency providers to meet the client’s needs along with supportive counseling. The Blended Case Manager will also provide advocacy in order to help consumers set and achieve recovery goals and empower consumers to increase self-advocacy.

Required Qualifications:

• 12 credits in coursework related to Human Services, Psychology or related field
• One or more years of direct clinical experience working with children, youth and families
• Manage a varied work schedule Monday-Friday between 8:30am - 8:30pm, including 3 required evenings per week

Preferred Qualifications:

• Bachelor’s degree in Human Services or related field
• Bilingual in Spanish.

Program Specific Responsibilities:

• Assist in the completion a comprehensive assessment and treatment plan for eachindividual client who has been referred for services.
• Assure all consumers on the assigned caseload receive an environmental matrix toidentify skill deficits and strengths to establish treatment goals.
• Complete a written strength assessment, develop and implement a written service plan for each consumer assigned to the caseload.
• Provide services that diffuse or resolve crises in order to prevent unnecessary use of emergency rooms, in-patient services and mental health court.
• Connect the consumer to behavioral health and social service agencies to assure there are no gaps in service.
• Make home, school, hospital and community-based visits on a daily basis. Must be able to travel throughout Delaware County to meet face to face with clients, families, schools, providers etc. as based on client needs.
• Utilize IPAD and cell phone provided to complete documentation tasks and outreach to clients, providers, coordinate resources etc.
• Familiar with and able to access community resources and child serving systems in Delaware County.
• Participate in weekly team meetings and supervision
• Assume case load of up to 30 clients and achieve billable hour requirements per week.
• Provide on call support for 24/7 mental health crisis support in a rotation with other program staff.
