The Veterans Health Administration, Department of Veterans Affairs, is seeking a Registered Nurse - Mental Health Care Manager for the outpatient Mental Health ICC. You will deliver Veteran-centered care, performing assessments, planning, interventions and evaluating treatment while coordinating with the broader health care team.

Qualificações

• English language proficiency is required for direct patient care.
• Graduate of a nursing program approved by an accrediting body and licensed in the United States.
• Current, full, active, and unrestricted registered nursing licensure in a U.S. state, territory, or the District of Columbia.

Responsabilidades

• This Registered Nurse is located in the outpatient clinic of the Mental Health (MH) Integrated Clinical Community (ICC).
• The RN MH Care Manager is an integral member of the interdisciplinary team and works to provide Veteran-centered mental health care.
• Delivers assessments, planning, interventions, evaluation of treatment and continuity of care.
• Maintains regular contact with Veterans and coordinates treatment plans with the care team.
• Acts as MH Treatment Coordinator and primary point of contact, facilitating resolution of identified needs.
