Responsibilities

• Coordinate care for a group of patients to achieve clinical outcomes.
• Manage hand-off communication and conduct rounds to familiarize self with patients.
• Oversee admission through nursing assessment and discharge with belongings.
• Provide one-to-one assistance to assigned patients.
• Supervise the therapeutic milieu and de-escalation techniques.
• Participate in department performance improvement activities.
• Assist with activities of daily living and self-care instructions.
• Execute directives provided by the charge nurse.
• Run Behavioral Health groups.
