Job summary

Southcoast Health in Fall River, MA, is seeking a Per Diem Mental Health Associate to support behavioral health services. This role involves day/evening shifts with weekend/holiday/on-call rotation at Charlton Memorial Hospital.

The position emphasizes equitable care, education, and collaboration within a team to meet patient plans. The role requires a high school diploma with related health work training; Bachelors preferred.

Qualifications

• Equal to completion of 4 years of high school plus the completion of additional courses/training in community health work or behavioral health is required.
• Bachelors degree preferred. Culturally competent with solid knowledge of the communities being served.
• Demonstrated ability to perform as part of a multidisciplinary team.
• Must have basic competency with documentation and computer skills.
• Community Health Worker and/or Recovery Coach certification preferred, as is experience with EMR in medical settings, and Bilingual skills.
• Must have fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
• Compensation: Pay rate will be determined based on level of experience.

Responsibilities

• Expand organizational support for equitable and accessible health management of behavioral health, substance use disorders, and social determinants of health (SDOH).
• Provide culturally appropriate health education and navigate systems.
• Facilitate coordination of care and community resources to meet the patient's plan of care.
• Work in a team-based role and participate in team activities.
• Promote patient autonomy, address barriers to support treatment recommendations and optimal disposition planning for patients with a range of behavioral health, social, and medical concerns.

Hours: Per Diem

Shift: Day/Evening shift with weekend, holiday, and on-call rotation; 8:00 am - 4:30 pm and 3:30 pm - 12:00 am
