• Collaborate with team managers, agencies, and families to create cohesive treatment plans addressing patient needs and goals.
• Empower clients and families by connecting them with community resources and guiding referrals.
• Build partnerships with community health, welfare, and social agencies to find innovative solutions for patient goals.
• Manage concurrent reviews, discharge planning, and post-discharge needs to ensure continuity of care.
