Qualifications

• Graduate of nursing program. Must hold active LPN license.
• Experience supervising nursing staff in a memory care or long-term care setting is preferred.
• Ability to read, interpret and apply regulatory standards and care plans.

Responsibilities

• Makes daily rounds on assigned unit to observe residents, verify information, monitor staff assignments and condition changes.
• Administers and monitors nursing care, vital signs, intake/output and medication administration as required.
• Develops and revises care plans with the interdisciplinary team to support residents’ goals.
• Ensures timely communication with residents, families and physicians about changes in condition and care decisions.
• Trains and develops staff, ensures proper documentation of care, and maintains regulatory compliance.
