Job summary

Hilltop Community Resources in Western Colorado is seeking a Memory Care Specialist to provide compassionate support to residents with dementia in our assisted living environment. The role emphasizes safety, dignity, and adherence to care plans while collaborating with a caring team.

The Memory Care QMAP pathway is available, with flexible part-time and full-time schedules and opportunities for professional development within a values-driven, inclusive workplace.

Qualifications

  • High School Diploma or GED; 1 year direct care with Alzheimer's preferred.
  • QMAP certification required for Memory Care Specialist/QMAP role.
  • Colorado Identification Card and Food Handler’s Card needed within specified timeframes.

Responsibilities

  • Provide personal care per care plan and document care.
  • Maintain resident safety, dignity, and rights; monitor environment.
  • Assist residents with ambulation, meals, and activities.
  • Respond to radio/pager calls and resident requests promptly.
  • Support memory care activities and social engagement.

Essential Duties And Responsibilities
  • Demonstrate ethical and professional behavior that align with the values of Hilltop’s Assisted Living Communities (ALCs), including Service 1, how we treat each other, and Service 2, how we treat our residents. Service 1 and Service 2 is demonstrated by setting boundaries, being predictable, consistent, kind, and transparent.
  • Understand and strictly follow all applicable Hilltop, Fountains, and state and federal rules, regulations, policies, and procedures.
  • Maintain professional boundaries with all residents, while maintaining the dignity, confidentiality, services, and rights of every client.
  • Providing protective oversight, to include assisting residents, maintaining a safe physical environment, redirecting and assisting disoriented or confused residents, applying and following house rules and honoring resident rights, appropriately reporting and assisting residents experiencing difficulties, and identifying and monitoring visitors to the facility.
  • Providing social supervision, including helping with transportation and ambulation to events, assisting with communication of and attendance to social events, and assisting with general socialization.
  • Providing for the sanitation and safety of residents, including monitoring the safety of the living and common areas, removing and reporting any safety hazards, maintaining safety supplies, redirecting residents away from unsafe areas or situations, etc.
  • Providing personal care to residents as directed by the specific care plan and appropriately documenting care given.
  • Monitoring ambulation and assisting residents to and from meals and/or activities.
  • Performing housekeeping duties to include but not limited to mopping, dusting, trash removal, linen change, stocking supplies, vacuuming, disinfecting/sanitizing, cleaning bathrooms, cleaning bedrooms, cleaning living rooms, cleaning wheel chairs and walkers, organization of apartment, and laundry.
  • Answering radio, pager, and pendant requests for assistance.
  • Assist residents with memory care activities and social supervision.
  • Order meals for residents and serve as needed, clean and sanitize the dining area.
  • Redirection and de-escalation of behavioral residents.

Education & Experience

High School Diploma or General Education Degree (GED) plus one year of direct care experience working with the Alzheimer’s population preferred. Experience providing personal care and assistance with ADL’s required. Minimum 18 years of age required. A valid QMAP certification is required for the Memory Care Specialist QMAP position.

CERTIFICATES, LICENSES & REGISTRATIONS
  • Colorado Identification Card
  • Food Handler’s Card (required within six months of employment)
  • Hilltop Dementia Certification (required within six months of employment)
  • CPR Certificate/First Aid Certificate (preferred)
  • Qualified Medication Administration Personnel (QMAP) Certification
  • De-Escalation Training (preferred)

    • The starting wage range for the Memory Care Specialist role is $17.85 to $18.74 per hour. The starting wage range for the Memory Care QMAP role is $18.35-$19.27 per hour.
