Job summary

Seaton Senior Living, LLC in Maryland is hiring a Memory Care SHINE Coordinator to support, develop, and implement Memory Care programs. You will provide physical and emotional support to Memory Care residents while maintaining a safe, home-like environment and delivering patient-centered care.

Collaborate with the Celebrations Director and DHW to design engaging activities, train staff, and ensure individualized dining and daily care plans are followed. Prior experience in healthcare required.

Qualifications

• One year of experience in a healthcare environment.

Responsibilities

• Designs, schedules, and facilitates Memory Care program incorporating Life Skills and dementia-appropriate activities.
• Partners with the Celebrations Director to ensure a variety of activities are available throughout the day and that Care Managers are engaged.
• Coordinates a comprehensive program of activities to meet the needs and interests of Memory Care residents.
• Supports the clinical team with in-service education related to disease processes and resident care.
• Refers clinical issues to the Director of Health and Wellness to address residents' needs.
• Before move-in, communicates resident preferences to Memory Care team.
• Acts as Memory Care program champion and coordinates with dining services to meet individual dining needs.
• Reports resident changes in condition to DHW and ED and updates Daily Log as appropriate.
• Collaborates with leadership on programming tailored to Alzheimer’s/Dementia care.
• Serves as the family point of contact for Memory Care residents.
• Maintains operations within state regulations.
