Job summary

Elderwood is seeking a Memory Care Program Specialist to develop and lead therapeutic activities for residents in a memory care setting. The role supports the Director of Activities and Memory Care to enhance residents’ physical and emotional wellbeing.

The ideal candidate holds a relevant degree or certification, with experience in long-term care activities and dementia care. Responsibilities include calendar management, care documentation, and collaboration with nursing staff.

Qualifications

• Bachelor’s degree in Recreational Therapy, Education or related field OR COTA/Associates with 1–2 years in long-term care activities.
• Current, valid state certification or license or willingness to obtain National Certification within 1 year.
• Certification in dementia and/or behavior management (e.g. CDT) or will obtain within 1 year of hire.
• Demonstrated computer skills including MS Office and web-based applications.

Responsibilities

• Provide therapeutic and social programming to meet residents' physical and emotional needs.
• Input and maintain daily memory care calendar, resident participation reviews, attendance and progress reports.
• Assist with meal service and feeding as trained.
• Notify Licensed Nurse of changes in residents' conditions or needs.
• Maintain facility standards for resident care and safety.
