Job summary

TerraBella Thornblade is seeking a Memory Care Coordinator to design and oversee the Memory Care program, coordinating with the Memory Care Director to deliver person-centered activities and ensure a safe, home-like environment for residents.

The role emphasizes ongoing staff education, resident engagement, and collaboration with dining services, with a focus on dementia care excellence and regulatory compliance to support TerraBella’s commitment to compassionate senior living.

Qualifications

• One year dementia care experience.
• One year in a leadership role.

Responsibilities

• Designs, schedules and facilitates Memory Care program with Life Skills and dementia-appropriate activities.
• Partners with Memory Care Director to ensure a variety of activities are available and that Care Managers are engaged.
• Provides ongoing in-service education to Memory Care staff on relevant disease processes and population served.
• Reviews resident preferences and needs before move-in and educates Care Managers accordingly.
• Facilitates Service Plan and Daily Assignment Sheet development to ensure excellent service delivery, safety, hydration and resident engagement.
• Acts as the community champion for Memory Care programming and activities.
• Partners with dining services to meet resident needs and preferences; participates in dining experiences.
• Reports resident condition changes to the nurse.
• Develops connections with local Alzheimer’s organizations and tailors programming to residents with Alzheimer’s or related dementia.
• Creates a therapeutic environment focused on first impressions and resident engagement.
• Develops plans to correct deficiencies and ensure regulatory compliance.
