Job Summary

In alignment with Brookdale's Resident and Family Engagement philosophy, the Program Coordinator plans, coordinates, and facilitates a calendar of person-centered dementia-friendly programs that meet the specific needs of residents within the dementia care community. The role involves providing meaningful and purposeful programs to enhance residents' quality of life, recognizing individual abilities and needs, and encouraging independence while honoring their habits, routines, interests, and preferences.

Essential Functions

  1. Designs a 7-day-a-week person-centered program that is engaging, creative, and tailored to residents' needs and interests.
  2. Promotes and engages residents in the 7 domains of well-being programs in a dementia-friendly environment, including sensory-focused small group activities for residents unable to participate in larger groups.
  3. Develops a monthly calendar in partnership with residents based on shared interests and ensures the scheduling and execution of special events and daily programs.
  4. Ensures compliance with Brookdale standards, regulations, and policies in dementia care programs.
  5. Collaborates with community leadership to educate and provide resources to families and staff regarding Alzheimer's and other dementias, including facilitating in-services and training programs.
  6. May supervise Clare Bridge Program Assistants, including hiring, coaching, and performance evaluations, and may act in supervisory capacity in the absence of the manager or director.
  7. Participates in planning and executing community events, including holidays and educational programs.
  8. Reports changes in residents' health and behavior to the clinical team and collaborates on interventions.
  9. Oversees social media content to ensure alignment with Brookdale's culture and standards.
  10. Plans and schedules external programming, possibly including driving community vehicles.
  11. Maintains department budget and manages volunteers.
  12. Performs personal care, housekeeping, and dietary duties as needed to maintain service quality, with proper training and qualifications.
