Position Overview:
As an Administrative Support Coordinator for the Dementia Program you will assist in delivering the 9 core elements outlined in the CMMI GUIDE Model of care delivery. You will build strong, collaborative relationships with internal teams as well as external partners to ensure patients and caregivers receive appropriate and high quality care. Directly supporting GUIDE Care Navigators with scheduling assessments, obtaining consent for services, providing resources and support to patients and caregivers, and coordination of care are foundational to the position.The position allows for great work-life balance being fully remote.

Schedule: Full time position, day shift hours, no evenings, weekends or holidays. Hours are 8am to 5pm Monday thru Thursday & 8am to 3pm on Fridays.

Location: This remote role must be located in one of the Bluestone Markets (Minnesota, Wisconsin or Florida).

Salary: $19.50 - $23.00. Salary will be commensurate with experience.

Responsibilities:
  • Administratively support Dementia Care Navigators
    • Data entry into care management patient registries
    • Scanning, uploading and management of documents in EMR (Electronic Medical Record) System
    • Prep assessments and other documents/forms
    • Outreach and scheduling visits for eligible GUIDE patients and/or their caregivers
    • Ongoing telephonic contact with patient and caregivers, community partners
    • Coordinate collaboration with other team members, primary care providers, HIM, CA, etc
    • Participation in Care Management meetings
    • Prepare GUIDE documents for the billing/coding department
    • Ensure documents are completed and submitted timely to Data Custodian
  • Helping connect patients and caregivers with resources and services
  • Completing required documentation
  • Collaborating with primary care teams to ensure high quality team-based care
  • Use utilization management tools to facilitate Care Navigator involvement in transitional care activities
  • Collaborate with hospitals, rehabs, and SNFs to manage patient’s inpatient stay and desired discharge plan
  • Work to increase coordination of care for a vastly complex geriatric population
  • Be proficient in community resources
  • Operates office equipment such as computers, phones, and printers to communicate and update electronic medical records and other databases as appropriate
  • Communicates and collaborates with multiple departments and other Administrative Associates
  • Ensures electronic medical records are accurately updated
  • Provides backup support to team members as needed
  • Works independently and within a team setting

Qualifications:

Education/Certification/Experience

  • Associate’s degree in Human Services, Health Care Administration, or a related field preferred but not required
  • Minimum of one year experience working in a health care related role
  • One year of experience working with EMR and/or Care Coordination software preferred
  • Experience working directly with the Dementia population preferred

Knowledge/Skills/Abilities

  • Ability to work independently
  • Strong customer service, relationship building, and communication skills
  • Strong technical skills and experience with EHRs preferred
  • Demonstrated compatibility with Bluestone’s purpose, focus and values
  • Ability to travel throughout the market area as needed
  • Demonstrated ability to read, write, speak, and understand the English language
