Job Summary

Assists the Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person-centered dementia-friendly programs that meet the specific needs of each resident within the dementia care community. In alignment with Brookdale's Resident and Family Engagement philosophy, helps provide meaningful and purposeful dementia care programs that enhance residents' quality of life. Recognizes individual abilities and needs, encouraging independence and honoring residents' habits, routines, interests, and preferences in care and programming.

Essential Functions

1. Supports the implementation of person-centered programs tailored to residents' needs and interests.
2. Assists in leading and engaging residents in the 7 domains of well-being programs—purposeful, intellectual, social, spiritual, emotional, and physical—in a dementia-friendly environment, including sensory-focused small groups for residents unable to participate in large groups.
3. Helps review individual residents' service plans and completes program documentation as per Brookdale standards and regulations.
4. Ensures dementia care programs comply with state, federal, and other regulations, aligning with Brookdale’s mission, policies, and standards.
5. Provides guidance and resources to staff on programming and engagement to improve residents' quality of life.
6. Coordinates and participates in community events such as family socials, marketing, and open houses.
7. Communicates positively with residents, families, and professionals regarding the dementia care program and residents' personal needs and successes.
8. Assists in meeting quality improvement initiatives, including the Brookdale Excellence Standards Tool (BEST).
9. Supports programming outside the community, including driving a community vehicle as needed.
10. Assists with volunteer programs, including training and supervision of volunteers.
11. Performs personal care, housekeeping, and dietary duties as needed to maintain service quality in accordance with program standards.
