Job summary

Hillcrest Grand Lodge Memory Care is seeking a dedicated Medication Aide and Personal Care Assistant to support residents with daily activities and safe, compassionate care at our Papillion location. You will assist with ADLs, medication administration, vital signs, documentation, and mealtime support, while engaging residents in meaningful activities.

The role requires a current Medication Aide credential, and prefers experience in memory care or assisted living.

Qualifications

• High school diploma or equivalent preferred.
• Completion of 40-hour Medication Aide coursework required.
• Experience in Assisted Living, Memory Care, Dementia care, senior living, or personal care preferred.

Responsibilities

• Assist residents with activities of daily living based on individual care plans.
• Support bathing, dressing, grooming, hygiene, mobility, toileting and other personal care tasks.
• Provide medication assistance following the five rights and document care accordingly.
• Take vital signs as ordered and report out-of-range results to the Nurse Manager or Director.
• Complete monthly summaries, weights, vitals, and assessments timely and accurately.
• Assist residents to dining for meals and help serve and support during meals.
• Engage residents in daily activities and meaningful moments beyond scheduled times.
• Observe changes in condition or behavior and prompt communication with the care team.
• Complete documentation in the electronic medical record, including DAR notes by exception.
