Job summary

Hillcrest Health Services is seeking a compassionate Personal Care Assistant – Memory Care to join the Grand Lodge Memory Care team in Papillion, NE. The role emphasizes medication assistance, daily living support, and meaningful engagement for residents.

The position requires a current Medication Aide and completion of 40‑hour coursework, with day and evening shifts available for full-time and part-time openings.

Qualifications

• High school diploma or equivalent preferred.
• Completion of 40‑hour Medication Aide coursework required.
• Previous Assisted Living, Memory Care, Dementia care, senior living, or personal care experience preferred.

Responsibilities

• Assist residents with activities of daily living based on individual needs, preferences, and care plans.
• Support residents with bathing, dressing, grooming, hygiene, mobility, toileting, and other personal care needs.
• Provide medication assistance following the five rights and accurately document medication support, as credentialed.
• Take vital signs as ordered and report out‑of‑range results to the Nurse Manager or Director of Health Services.
• Complete monthly summaries, weights, vitals, and assessments timely and accurately.
• Assist residents to the dining room for meals and help serve, encourage, and support residents during mealtimes.
• Engage residents in daily activities, scheduled programming, and meaningful moments outside of scheduled activities.
• Observe residents for changes in condition, behavior, needs, or concerns and communicate promptly with the nurse and care team.
• Complete documentation in the electronic medical record, including DAR notes by exception.
• Wash, put away, and complete assigned resident laundry, including incontinent laundry as needed.
• Monitor resident supplies and notify family as needed.
• Support a clean, safe, calm, and comfortable Memory Care environment.
• Build positive relationships with residents, families, nurses, PCAs, and team members.
