University of Minnesota Physicians (M Physicians) seeks a Physician Assistant or Nurse Practitioner to join the Memory Care team in an outpatient setting at our St. Louis Park location.

You will conduct cognitive assessments, develop care plans with neurologists, and educate patients and caregivers on disease progression and treatment options. You will collaborate with an interdisciplinary team, respond to urgent neurological issues, and participate in quality improvement initiatives within

Qualifications

• Graduate of an accredited school of nursing with formal NP training or graduate of an accredited PA program.
• Current MN RN license with prescriptive privileges OR PA license with MN prescriptive authority.
• Certified through ANA/AANC or eligible for exam; or through NCCPA for credentialing and privileges.

Responsabilités

• Perform comprehensive cognitive assessments including history-taking and neurological exams.
• Develop and adjust individualized care plans with memory care team.
• Educate patients and caregivers on disease progression and treatment options.
• Respond to urgent neurological issues related to treatments and triage steps.
• Participate in interdisciplinary case conferences and quality improvement in memory care.
• Collaborate with care team, patients, and families on ongoing basis.
• Full-time 40 hours per week; Monday–Friday; no current weekend/holiday requirements.
