Job summary

Hollenbeck Palms seeks a full-time LVN for the Memory Enhancement Center in Los Angeles, CA, with night shifts 11:00 pm–7:00 am and a strong benefits package. You will coordinate resident health care activities, assess needs, and help ensure high-quality care for residents, including those with memory impairment.

You will administer medications, develop care plans, monitor changes, and collaborate with physicians, families, and care staff.

Qualifications

• Must possess a valid California LVN license.
• Demonstrated dedication to senior care with excellent customer service.
• Positive attitude, flexibility and teamwork are essential.
• Experience in long-term care, geriatrics, or memory care preferred.
• English reading/writing/speaking required.
• CPR/BLS certification.
• Criminal background check required.

Responsibilities

• Support the clinical and care needs of residents.
• Medication administration; ordering and refilling medications, dispensing to residents.
• Provide compassionate care to residents with Alzheimer's/dementia.
• Coordinate with physicians and families; act as liaison.
• Develop and update comprehensive care plans as needs change.
• Monitor vitals and changes; maintain electronic records in PCC.
• Assist with admissions, discharges, and transfers as needed.
• Follow infection prevention protocols and safety procedures.
