Job description

Smithfield, Virginia – Riverside Lifelong Health & Rehabilitation (Smithfield). Memory Care or Assisted Living. Position: Full‑time. Days: Memory Care. Some weekend shifts may be required. Day Shift sign‑on bonus: $2,500. Night Shift sign‑on bonus: $6,000.

Overview

Works under a Registered Nurse, Nurse Manager or Nursing Supervisor, and ultimately reports to the Director of Nursing or Assistant Director of Nursing and Director of Quality Care Management. The Licensed Practical Nurse (LPN) monitors residents, participates in the planning, implementation and evaluation of nursing care, and provides emotional, physical comfort and safety to residents and their families. The LPN assists in the supervision of auxiliary personnel.

Responsibilities

• Administer medications and treatments accurately and according to provider’s orders.
• Allow resident choices within limits of capability, considering safety and dignity issues.
• Assign duties to non‑professional staff such as CNAs according to education, training, policy and resident need to assure quality resident care.
• Engage team members for optimal performance and address performance as needed.
• Participate in administering medications, treatments, monitoring and observation of resident care.
• Identify changes in condition with proper documentation according to standards of nursing practice.
• Demonstrate consistent follow‑through to promote positive resident outcomes.
• Utilize good judgment and specialized skills.

Requirements

• LPN diploma from an accredited program required (only for new grads within 90 days of program completion).
• Certification: CPR/BLS (within 30 days of hire) required; FA (within 30 days of hire); LPN license in the state of VA or compact required or board eligible.
• Primary source verification required only for new graduates without their license within 90 days of graduation.
