Job summary

Gentle Touch Health Initiatives in Plymouth, MN is seeking a Memory Care Coordinator to create warm, meaningful days for residents living with dementia. You will be the go-to for families and the care team, delivering engaging activities and personalized care.

We seek creative, compassionate professionals with 1–2+ years in dementia care or senior living. Competitive pay, benefits, and ongoing training are offered, along with a supportive leadership team.

Qualifications

• 1–2+ years of experience in dementia care, activities, or senior living.
• Patience, creativity, and a sense of humor.
• Clear, compassionate communication with families, including in hard conversations.

Responsibilities

• Get to know each resident’s abilities, history, and interests to tailor programming.
• Plan and lead a wide variety of recreational, therapeutic, and social activities.
• Create and maintain profiles for each resident for personalization.
• Foster an atmosphere of engagement, security, and comfort through innovative programming.
• Work alongside Resident Assistants and volunteers to bring daily activities to life.
• Co-facilitate monthly Alzheimer’s/Dementia Support Group with the Life Enrichment Director and the Nurse.
• Attend and co-facilitate resident care conferences with the nurse to discuss resident needs and care plans.
• Plan and oversee family events that encourage interaction and engagement.
• Serve as a resource and point of contact for families, offering education and support.
• Keep families informed with regular updates, including photos and good moments, not just concerns.
• Partner with nursing to let families know promptly about changes in mood, engagement, or behavior.
• Manage memory care activity supplies within established budget. Purchase needed supplies under established protocols.
