Job summary

Nhccare is seeking an RN/LPN Director of Memory Care for The Palmettos of Bluffton in Bluffton, SC. The role oversees the memory care unit’s programs and services, aiming to fulfill residents' psychological, intellectual, physical, social, emotional, and spiritual needs while promoting quality of life in a nurturing environment.

Responsibilities include supervising daily operations, conducting assessments, updating care plans biannually, and leading care plan meetings.

Qualifications

• Bachelor’s degree in related field; Alzheimer’s and Dementia experience preferred in a residential setting.
• Strong understanding of activities professional has completed a state-approved training course.

Responsibilities

• Supervise daily operation of the memory care unit.
• Perform pre-move in assessments with Nursing/Marketing collaboration.
• Initiate and update resident care plans every six months; revise as needed.
• Coordinate and participate in care plan meetings.
