About the Role:

As a Memory Care Coordinator, you will be responsible for coordinating and delivering personalized memory care services to our residents. This includes:

• Developing and implementing individualized care plans in collaboration with the care team
• Providing emotional support and guidance to residents and their families
• Facilitating resident activities and socialization programs
• Collaborating with caregivers and other healthcare professionals to ensure seamless care transitions
• Maintaining accurate and up-to-date records of resident care and services
