Job summary

Cogir Senior Living in Oregon is seeking a Resident Care Coordinator to serve as liaison among residents, families, and the care team, supporting the Health and Wellness Director with scheduling, staff supervision, and quality assisted living services onsite.

You will coordinate admissions, manage medication administration, maintain resident records, and ensure compliance with state regulations while delivering compassionate, respectful care.

Qualifications

  • Two years of experience administering medications in a professional caregiving setting.
  • One year of supervisory experience, including scheduling.
  • Memory care experience preferred.
  • Excellent communication and teamwork.

Responsibilities

  • Schedule and assign caregiving and medication tech shifts to ensure coverage and guidelines.
  • Participate in recruitment and interviews for new care team members.
  • Supervise, train, and support care staff, promoting teamwork and standards.
  • Oversee medication room and orders, collaborating with technicians.
  • Maintain accurate resident records, care plans, and medical documentation.
  • Coordinate care planning with home health agencies on-site.
  • Coordinate admissions with the sales team and supervise move-ins.
  • Conduct family/resident admission interviews.

POSITION SUMMARY

Our Resident Care Coordinator serves as a liaison between residents, their families, and the care team, promoting a supportive and safe living environment. They support the Health and Wellness Director by assisting in scheduling, supervising staff, and ensuring that systems are in place to provide quality assisted living services in all resident care areas. Duties include problem-solving resident concerns and coordinating care with the team and external providers. Additionally, they help with medication administration and direct resident care when necessary.

KEY RESPONSIBILITIES

  • Schedule and assign caregiving and medication tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
  • Participate in the recruitment and interview process for new care team members.
  • Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
  • Supervise the medication room and orders, collaborating with medication technicians and caregivers.
  • Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
  • Coordinate care planning with home health agencies on-site, working within community and company policies.
  • Coordinate admissions with the sales team, including supervising move-ins.
  • Complete family/resident admission interviews.
  • Facilitate effective communication among residents, their families, and healthcare providers to ensure seamless care delivery.
  • Address residents' special needs, requests, and complaints.
  • Assist with medication administration and provide direct care to residents as needed.
  • Understand the community's care regulations, as well as state-specific regulations.
Requirements
CANDIDATE QUALIFICATIONS
Education and Certifications:
  • High School Diploma or equivalent.
  • Must have a valid state medication administration license (such as CNA/CMA/CMT/QMAP) or meet the state requirements for medication administration in assisted living.
  • An LPN/LVN nursing license is a plus.
  • Current First Aid and CPR license or ability to obtain.
Experience, Competencies, and Skills:
  • At least two (2) years of experience administering medications and providing care in a professional caregiving setting is required.
  • At least one (1) year of relevant supervisory experience, including staff scheduling, is preferred.
  • Experience with memory care is preferred.
  • Excellent attention to detail and high integrity.
  • Excellent communication skills and teamwork mindset.
  • Strong leadership and team management skills.
  • A positive attitude, empathy, patience, and a commitment to treating our residents with dignity and respect.
  • Willingness to participate in weekend rotation.
