Job summary

Merrill Gardens at Cedar Mill is seeking a Memory Care Coordinator to support memory care residents through coordination and delivery of personal care services. This role assists with move-ins, assessments, and care planning, and acts as a key liaison between residents, families, and staff.

The coordinator will monitor and document resident conditions, ensure accurate records and regulatory compliance, and provide hands-on personal care while promoting quality outcomes.

Qualifications

• High school diploma or GED required; college degree preferred.
• 2+ years of experience in senior living, especially memory care or assisted living.

Responsibilities

• Supporting memory care residents through coordination and delivery of personal care services
• Assisting with resident move-ins, assessments, and care planning
• Monitoring and documenting resident conditions and service needs

Physical Requirements:

This role involves frequent standing, walking, lifting (up to 75 lbs), and assisting with resident care. Regular bending, reaching, and physical movement are required throughout the workday.
