Job summary

Gentle Touch Health Initiatives in Minnesota is seeking a Memory Care Coordinator to create warm, meaningful days for residents living with dementia. You will be the go-to for families and the team, fostering engagement and comfort through personalized programming.

Ideal candidates have 1–2+ years in dementia care, activities, or senior living, with patient, creative, and compassionate communication. The role offers growth, training, and a supportive leadership team.

Responsibilities

• Plan, prepare and lead recreational, therapeutic, and social activities to enhance residents' well‑being.
• Create and maintain profiles for each resident for personalized experiences.
• Co-facilitate Alzheimer’s/Dementia support groups with the Life Enrichment Director.
• Attend and co-facilitate resident care conferences with the nurse.
• Manage memory care activity supplies within budget and document attendance of events.

What you'll do

Create Meaningful Days

• Get to know each resident's abilities, history, and interests, and develop customized programming around them
• Plan, prepare and lead a wide variety of recreational, therapeutic, and social activities to enhance residents' well-being
• Create and maintain profiles for each resident to ensure personalized experiences
• Foster an atmosphere of engagement, security, and comfort through innovative programming
• Work alongside Resident Assistants and volunteers to bring daily activities to life

Family and Community Support

• Co-facilitate monthly Alzheimer's/Dementia Support Group with the Life Enrichment Director and the Nurse. Provide information and resources for support group attendees
• Attend and co-facilitate resident care conferences with the nurse to discuss resident needs and care plans
• Plan and oversee family events that encourage interaction and engagement
• Serve as a resource and point of contact for families, offering education and support
• Keep families informed with regular updates, including photos and good moments, not just concerns
• Partner with nursing to let families know promptly about changes in mood, engagement, or behavior

Administrative

• Manage memory care activity supplies within established budget. Purchase needed supplies under established protocols. Make recommendations for expenses during annual budget planning
• Adhere to all company personnel directives as per the Employee Handbook
• Document attendance of events
• Other duties as assigned

What you bring

• 1-2+ years of experience in dementia care, activities, or senior living
• Patience, creativity, and a sense of humor
• Clear, compassionate communication with families, including in hard conversations

Physical Requirements (based essential responsibilities)

Must be able to frequently perform tasks such as, but not limited to, those listed below:

• Stand, sit, bend, and squat while leading activities and working with residents
• Occasionally lift, push, or pull items weighing 10-25 lbs
• Walk, stoop, and carry objects throughout the day
• Communicate clearly face to face and by phone, including with background noise
• Use hands and arms for simple grasping, pushing and pulling in using supplies for activity programs

Environment

Works indoors in a climate-controlled environment majority of the time. Works outdoors in elements of seasonal weather for periods of time to conduct trips
