Job summary

Sycamore Heights Health and Rehabilitation is seeking a compassionate Memory Care CNA for our Memory Care Unit in Louisville, KY. This role focuses on providing person‑centered care to residents with Alzheimer’s and memory‑related conditions, fostering dignity and independence.

You will support ADLs, monitor changes, and work closely with licensed staff to ensure a safe, calming environment. Join a dedicated team that values collaboration, empathy, and professional growth in a rewarding setting.

Qualifications

• Current CNA license in the state.
• At least one year of LPN experience in a long‑term care setting is preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Provide hands‑on care to residents, including assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
• Support residents with memory‑related conditions using patience, redirection, and reassurance techniques.
• Assist with mobility, transfers, and ambulation to ensure resident safety.
• Monitor and report changes in condition, behavior, or mood to the nurse.
• Obtain and record vital signs (blood pressure, pulse, temperature, and respiratory rate) as directed.
• Assist with meal service and feeding, while encouraging independence when possible.
• Maintain a safe, calm, and structured environment for residents.
• Provide emotional support and companionship, building meaningful relationships with residents.
• Assist with incontinence care and maintain proper hygiene.
• Ensure accurate charting and documentation of care provided.
• Maintain confidentiality of resident information at all times.
• Communicate effectively with nurses, team members, residents, and families.
• Follow the direction from licensed nursing staff and ask questions when clarification is needed.
• Support a positive team environment and serve as a role model for peers.
• Provide excellent customer service to residents, families, and visitors.
• Adhere to all facility policies, procedures, and infection control guidelines.
