Job summary

Everbloom Memory Care in Salem, OR seeks an experienced RN to serve as Director of Clinical Services. This hands-on leadership role partners with the Administrator to move the clinical department forward, maintain survey-ready systems, and guide care teams through admissions and daily operations.

The ideal candidate will own the clinical assessment and admission process, supervise med tech staff, provide on-call guidance, and drive resident safety and outcomes as census grows toward 30

Qualifications

• Strong leadership of a clinical team in memory care or long-term care settings.
• Experience with resident assessments and admission processes.
• Knowledge of medication systems, MARs and documentation.
• Experience educating or supervising clinical/care staff.
• Excellent organization and time-management abilities.
• Strong clinical judgment and communication with families and professionals.
• Ability to work independently and take initiative.

Responsibilities

• Provide leadership and oversight of resident clinical care.
• Maintain survey-ready clinical systems and compliance.
• Monitor changes in resident condition and ensure timely intervention.
• Coordinate with physicians, pharmacies, hospice, and home health.
• Ensure accurate orders, documentation, assessments, and follow-ups.
• Identify clinical risks and implement solutions.
• Participate in QA and compliance activities.
• Serve as primary clinical resource to Administrator and care team.
• Own clinical deadlines and responsibilities without routine follow-up.
