Job summary

Kendal at Hanover is seeking a Care Manager - Memory Care (RN or LPN) to provide leadership and oversight for resident care coordination, documentation quality, and care plan development with a primary focus on the Assisted Living Level 1 and Memory Care Neighborhoods.

This role integrates clinical supervision and care management with a focus on compliance, interdisciplinary coordination, and high-quality resident-centered care.

Qualifications

• Hold current LPN or RN licensure in the State of New Hampshire and be in good standing without provisions.
• Have a minimum of two (2) years of clinical nursing experience with a preference for post-acute care, geriatrics, or care management.
• Demonstrate ability to communicate with all ages, especially geriatric population.
• Be able to walk, stand, bend, and move up to 50 pounds.

Responsibilities

• Care planning, conducting care conferences, order entry, clinical triage, appointment coordination, family communication, and documentation audits.
• As Nurse Supervisor on duty, provide supervision and support to nursing staff and may perform direct nursing tasks including medication administration and wound care.
