Job summary

Kendal at Hanover is a non-profit Continuing Care Retirement Community caring for 400 residents. The Community Care Manager (RN or LPN) provides care coordination, oversees transitions in care, and supports nursing staff, including medication administration and wound care.

The role requires NH RN or LPN licensure, a minimum of two years' nursing experience, and strong geriatric-focused communication. Hours are Monday–Friday 8:00 AM–4:30 PM, with on-site duties and a comprehensive benefits

Qualifications

• Hold current LPN or RN licensure in the State of New Hampshire.
• Minimum of two years of clinical nursing experience.
• Experience with geriatrics or care management preferred.

Responsibilities

• Care Coordination, including identifying clinical needs and coordinating services and placement.
• Resident Development and Community Outreach, engaging with residents and wellness visits.
• Documentation and Compliance, maintaining medical records and NH state compliance.
• Clinical and Supervisory Support, supervising nursing staff and providing guidance and training.
