Job summary

Modena Green Oaks in Illinois is seeking a Memory Care Assistant Director of Nursing (ADON) who is clinically strong, compassionate, and committed to supporting high‑quality resident care while leading our nursing team.

This role oversees direct nursing care, medication administration, care plan development, and supervision of staff; you will collaborate with physicians and a multidisciplinary team to maintain Solera quality standards and Residents’ Rights.

Qualifications

• Active LPN/RN license in good standing.
• Minimum 2 years nursing experience; prefer senior living/long-term care.
• Previous leadership or supervisory experience preferred.
• Knowledge of medication administration, care planning, and regulatory compliance.
• Strong communication, organization, and clinical judgment skills.
• Ability to work flexible schedules including evenings or weekends.

Responsibilities

• Provide direct nursing care and oversee care delivered by assigned team members.
• Administer medications, treatments, and procedures; complete required documentation.
• Monitor resident health status and respond to changes in condition.
• Assist with care plan development, reassessments, and resident move-ins.
• Supervise, coach, and evaluate assigned team members.
• Communicate daily updates to the Community Wellness Director.
• Coordinate care with physicians and interdisciplinary teams.
• Respond to emergencies and ensure safety and regulatory compliance.
• Maintain accurate resident records and uphold Residents' Rights.
• Promote a culture of hospitality, professionalism, and teamwork.

Position Overview

The Memory Care Assistant Director of Nursing (ADON) supports the Community Wellness Director by providing direct nursing care, supervising nursing team members, and ensuring resident care meets physician orders, regulatory requirements, and Solera quality standards.

Required Qualifications & Licensure

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing (state-specific)
• Minimum 2 years of nursing experience, preferably in senior living, assisted living, or long-term care
• Previous leadership or supervisory experience strongly preferred
• Knowledge of medication administration, care planning, and regulatory compliance
• Strong communication, organization, and clinical judgment skills
• Ability to work flexible schedules as needed, including occasional evenings or weekends
