Job summary

Heritage House Rehabilitation & Healthcare in Indiana is seeking a Memory Care Activities Assistant (Part-time, Days) to enrich residents' daily lives through engaging activities and meaningful interaction.

You'll work with the care team to create a warm, inclusive environment, tailor activities to memory-impaired residents, and provide compassionate support consistent with CARE values.

Qualifications

• High school diploma or GED; or 1–3 months’ related experience and/or training; or equivalent combination of education and experience.
• Successful completion of a State approved Nursing Assistant training and competency evaluation program or a CNA program approved by the State.

Responsibilities

• Plan and lead memory care activities that are engaging and age-appropriate.
• Provide compassionate, person-centered interactions to residents.
• Collaborate with the care team to support residents holistically.
