Job summary

Grey Stone Health & Rehab in Fort Wayne, IN is seeking a Memory Care Activity Assistant to join our full-time, on-site team. This role focuses on enriching residents' daily lives with warmth, patience, and meaningful activities, with CNA preferred and licensing pathway support.

You will collaborate with our care team to plan age-appropriate, memory-friendly programs, monitor residents' engagement, and promote safety and well-being throughout the day.

Qualifications

• High school diploma or GED; or 1–3 months related experience or training; or equivalent
• Successful completion of a State-approved Nursing Assistant training or CNA competency program (or community-provided)
